Dear all,
As you all know, the deadline and also the extended deadline had been way over. Instead email from Ms Piraveena, I am also writing for the same reason. For those who have yet to submit the reply form please do so. If it is impossible for you to get the signature, you could fill in the form on behalf of the doctor provided they had agreed.
Attached are lists of names:
1) who have yet to sumbit the form
2) submitted (red highlited means date clashes with other rotation)
I would be so grateful if these students in category 1 could be responsible enough to settle the matter. You had been briefed on what you should do unless you purposely skipped the briefing and have no alternative to ask.
Nevertheless, the Class Rep could come to collect the below from me on 14/12/2009 at AAD:
1. Study Guide for students
2. Introduction Letter to GPs
3. Guidebook for GPs
4. Assessment Form for GPs to sign
I also received forms (10pcs) with no name written. Don’t worry, u could just give me the GP name/clinic and also your details (including tel. no) in the GP tray.
