Guys,

1. We need to submit 1 case summary online, besides the log book which contains 10 cases. So there is a total of: 1 case report, 1 case summary, 1 case formulation, 1 completed log book (x 10).
   
   
2. For the format for case formulation you can refer to the Oxford textbook, and learning issues (minimum 2) are needed for all reports, formulation and summary.
   So far as I know there are some points you need to include into your case formulation
   

   1. Priority (c/c)
   2. HOP
   3. Factors (predisposing, precipitating, perpetuating)
   4. MSE & PE
   5. Learning issues

3. Case report format
   

   1. Detail history ( c/c, hop, psych & med Hx, family Hx, personal Hx, social Hx )
   2. PE ( MSE & PE )
   3. provisional Dx + d/d
   4. Ix + final diagnosis
   5. Management
   6. Discussion
   7. learning issues

4. The exam timetable will be put up on the notice board, so please kindly refer there for the latest updates. The groupings for submission of reports has been out, who ever still doesn't know whom to submit to, please find out from me.

Psychiatry Timetable Changes

Dear all,

For next week, group 1 is going for all the child psych clinics, group leaders please kindly take his phone number from me, thanks. For the next two weeks, group 1 & 3 will be going to Dr. Shane's teaching clinic.

Monday:

• Morning TBL 4 at 10am by Dr Zainab in MLT (Group 3 is in charge).
• Afternoon CP by Dr. Agnes at 2.30pm. Venue (I'll inform you guys on Monday morning during tbl session).
• Seminar shifted to Tuesday afternoon at 4pm. Venue waited to be confirmed.

Tuesday:

• Morning TBL 5 by Dr Philip has shifted to friday morning 11.30am. Venue yet to be confirmed. OPD group 3 has to go.
• Afternoon 4pm seminar X by Dr Selvi, venue yet to be confirmed.

Wednesday - follow time table because there're no changes.

Thursday:

• There might be an additional class in the afternoon by Dato Siva on ethics. Will confirm with you guys ASAP, or else follow the time table.

Friday:

• Morning 9am seminar by Dr. Nor with CP on Somatoform Disorder (Group 4 is in charge).
• 11.30am TBL 3 by Dr Philip (Group 2 is in charge).

Psych members,

This week child's psych clinic is incharged by medical officers in the clinic, and Grp 4 is supposed to go on both days (tues and wed)... For tmr's seminar, it is at 4pm incharged by Grp 1. Wed's seminar is incharged by grp 2. the venue for tmr is yet to be confirmed. but mostly is in one of the SRs or MLT.

The groupings :

Group 1 >> Roshnee, June, Cheryl, Zia, Rebecca, Chong Sen

Group 2 >> Chin Nam, jon, Koon Hau, Kokila, Izzat, Ee Wei

Group 3 >> Navin, Shuz, Reza, Joel, Jasmine, Sufi, Mira

Group 4 >> Fatin, Zaim, Karbela, Hamizah, Melvyn

During Dr. Zainab's and Dr. Shane's clinic session, the groups will be combined together 1 + 3 and 2 + 4

During ward work, each combined groups will take turns in male and female wards for 2 weeks. 1st and 2nd week is 1 + 3, then rotate after 2 weeks.

This week's and next week's Dr. Shane clinic session grp 2 + 4 has to go.
