Please take note that students are required to bear the cost of transportation for the community clinic visits during the Cardiovascular System Course.
However, those who are going for the community clinic visit in Nilai and Mantin are allowed to claim for transportation cost based on the following criteria:
(a) Students are required to car-pool. Only two cars are allowed to claim at any visit i.e. RM25 plus toll charges per car/day (you must provide toll tickets for claim). For claim, please indicate the following:
· Date of visit and clinic you have attended.
· Car number plate.
· Name and ID number of student who’s driving and telephone number.
· Name and ID number of students who car-pool.
· Toll tickets.

Please pass your claim to Ms Punitha in Academic Affairs Department latest by 6 February 2009. Claims after the stipulated date will not be accepted.
(b) No other modes of transportation to Nilai and Mantin Clinics are allowed to claim.
